Scheduling Appointments:
Appointments are scheduled by physician referral or self-referral.

What to bring to your first appointment:
Patients are encouraged to send or bring copies of records and reports that may be of help in the evaluation. The patient must bring all outside MRI studies, either on film or on a CD, of the brain and/or spine to their appointment.
Physician Referral:
The process of referral begins with contact from the primary care provider to the Chiari Treatment Center. The patient must have had a recent MRI of the brain or Cervical spine that shows the Chiari malformation. Patients diagnosed with Syringomyelia are also evaluated at the Chiari Treatment Center. Patients will be scheduled at the first available appointment time.
Self Referral:
For patients who choose to self-refer, an MRI of the brain (or cervical spine) that shows evidence of Chiari I Malformation is required. Depending on your insurance, you may need a referral from your primary care provider. It is best to check with your insurance company regarding their requirements for referral. We cannot accept out of state Medicaid. You must bring the MRI films (or CD) along with copies of any records and reports that may be of help in the evaluation.
